Admission to School A birth certificate, immunization record, Social Security Card, name and mailing address of previous school, proof of residency (*electric bill or phone bill) and two names and phone numbers of people to call in case of an emergency are all required to be admitted to Bonneville Elementary. Bring all documents for school registration.
Immunizations Utah law requires that school records include complete immunization records. Records must include documentation of the month, day and year for each immunization and are required before admission to school: DPT-5, Polio-4, (the fifth DPT and fourth Polio need to be given after the child's fourth birthday), Hepatitis B- 3 shots (for kindergarten through fourth grade students), Hepatitis A- 2, MMR- 2(MMR must be given after the first birthday) and Varicella (Chicken Pox) vaccinations (required for all kindergarten students or the date the child had Chicken Pox). TB is required for students coming from a foreign country or from certain states. Check with the school nurse if you have any questions regarding immunizations.
Accidents Whenever a serious accident or illness occurs at school, we will do everything within our power to contact a parent. If this fails, we notify the friend or neighbor listed on your health sheet. In extreme emergencies when we are unable to contact these people, we call the paramedics. It is critical that parents keep the office up-to-date with current home, work and cell numbers so parents can be reached in the event of an emergency.
Medication Utah State law prohibits any employee in a public school setting to administer over-the-counter pain relievers (Tylenol/Advil) without parental permission. If your child needs pain medication a parent will be called before medication is given. Other over-the-counter medications and prescription medications CANNOT be given to a student without a signed statement from a physician. A complete written request form must be on file with the school secretary.
Closed Campus For the safety and welfare of students and in compliance with school board policies, we maintain a closed campus. Students should not leave the school grounds from the time that school begins in the morning until school ends in the afternoon without permission from the principal. Students may go home for lunch with a written note from a parent. Students may not accompany friends home for lunch.
Classroom Visits Parents are welcome to visit the school. For the security and safety of our students, we ask that when visiting or volunteering in the school you check in at the school office to receive a visitor's pass. Occasionally out-of-town cousins or friends come to visit during the school year. These guests are not to accompany students to school. Child guests are often a distraction and are a burden to both the teacher and classmates.
If You Move Away Please notify the office and the classroom teacher at least one week in advance of moving away from Bonneville Elementary. Be sure all library books, school texts and materials are returned to teachers and staff.
Emergencies and Disasters In the event that an emergency occurs where our building cannot be occupied due to fire or utility failure, students will be evacuated to the LDS church building just west of the school on 1200 North until such time as they can be returned to our building. If an earthquake or other major disaster structurally damages the school building, students will remain on the school grounds under the care of the faculty and staff until they are safely released to their family or other individuals designated on their health information sheet.
Bikes, Scooters, Skateboards & Roller Blades Children may ride bikes and scooters to school. Bikes and scooters must be placed in the racks located on the west and east sides of the school. The school cannot be responsible for bike security. We suggest that students lock bikes and scooters to prevent theft. Scooters are considered the same as bikes and must not be brought in the building. Bikes and scooters are NOT to be ridden on school grounds before, during or after school. Skateboards and roller blades are NOT allowed at school.
Book Orders When sending money to school for book orders, checks MUST be made out to the individual book companies. Cash is not accepted for books orders. Do not make checks out to individual teachers or to the school for book orders. Checks will not be cashed by Bonneville Elementary.
Donations Basic donations may be made to schools by parents as long as all students have the opportunity to participate in field trips, lyceums, educational materials related to the curriculum and craft-type materials that students will retain. No student will be excluded from activities if they are unable to make a donation. Other donations for materials, for special gifts or projects may be requested by teachers. These special projects, such as Christmas or Mother's Day gifts, are optional activities. The school has a small budget to cover transportation costs of field trips but we do ask for help from parents to cover the admission costs to museums, plays and concerts. We will try to keep these requests to a minimum, but ask that you contribute if possible.
Dress Code Children should arrive at school appropriately dressed for the weather of the day. All healthy children are expected to go outside for recess and after eating lunch except when weather conditions are extreme. If you desire that your child be allowed to stay inside at lunch due to illness, please inform the teacher of your request in a note.
Dress Guidelines ● All children should dress modestly and appropriate for the weather. ● No short-shorts or bike shorts. Knee shorts are acceptable. ● Hats or other headgear should not be worn indoors. ● Tank tops or bare midriff shirts or blouses are unacceptable. ● The following clothing options are never acceptable at school: ► Clothing with vulgarities, profanities, obscenities ► Clothing with advertisements for alcohol, tobacco or drugs ► Clothing which is ragged, tattered or deliberately distracting in appearance
Drug Policy Alpine School District policy prohibits any use, distribution and/or selling of drugs or imitation drugs on any school campus. Doing so may result in expulsion of any student involved and the appropriate law officials will be notified. State Law prohibits any smoking by students or patrons on school grounds.
Homework We believe that the benefits of homework are extremely valuable. Communication between parents and teachers is essential in determining the appropriate amount of homework for your children. Homework requirements may vary from teacher to teacher and from student to student. Homework may be assigned on a daily basis to most upper grade students. We realize that children have other responsibilities, such as music lessons and scouting. If you have concerns about the amount of homework your child may have, please contact the teacher directly and in a timely method.
Lost and Found To simplify the return of lost items, we ask that all caps, jackets, mittens, boots, etc. be labeled with your child's name. Lost items are placed in the lost and found container near the lunchroom. Valuable or tiny articles are to be turned into the office. We encourage students to be responsible for their own personal items. Lost items not claimed will be donated to charity.
Recess ● All students are expected to be outside during recess breaks. ● Please dress your child for weather conditions (jacket or coat, gloves, hat). ● Inside days will be announced during extreme weather conditions (chill factor, rain or snow). ● Children stay in their classrooms for recess on bad weather days with productive activities. ● Check the lost and found frequently for lost items. Remember to label items with names. ● Children will be allowed to remain inside because of health problems on air quality days. ● If your child needs to stay in during recess for health reasons, notify the teacher in writing. ● Send a note from a doctor or contact the school nurse if your child needs to stay inside for recess for an extended period of time (a week or more). ● Students should know the route you want them to take to and from school. ● Cross the street only with a crosswalk and/or the crossing guard. ● Students should obey crossing guard and traffic laws at all times. ● Students should never run in front of or between cars and buses. ● Students should not be walking unaccompanied in the parking lot. ● Impress upon students not to talk to or accept rides from strangers. ● Encourage students to come directly home after school taking a safe and direct route.
Toys at School Please do not allow children to bring toys of any kind to school. This includes radios, pocket knives, squirt guns, roller blades, skateboards, dolls, etc. Such items found at school will be taken and returned ONLY to the parent. They distract the children and do not enhance the educational process.
Pets at School Please do not allow your children to bring their pets with them to school unless they are for Show & Tell. In that case, parents need to come with them so the pet may be taken home immediately thereafter.
Cell Phones Because of the disruptive nature of cell phones to classroom instruction, students are not allowed to have cell phones or other electronic devices switched on during school hours. |
